Are there any limitations I should know about?

How do I get reimbursed for the FreeStyle Libre 2 system?

First, talk to your healthcare professional to get a prescription. If you qualify for reimbursement under Yukon Health and Wellness, your healthcare provider will submit a Special Authority request to Yukon Health and Wellness on your behalf. Upon approval of your request, your claim for the FreeStyle Libre 2 system will be processed like any other claim at the local pharmacy.

Are there any out-of-pocket costs?

There is an annual deductible with this program. For more information, send an email to yukon.healthcare@yukon.ca.

Can I get reimbursed if I’ve already paid for the FreeStyle Libre 2 system?

If you purchased the FreeStyle Libre 2 system before registering for the Chronic Disease and Disability Benefits Program, you’ll need to submit a claim within a year of purchase. Along with your claim, you’ll need to include:

  • Documentation of the purchase, and
  • Documentation of the recommendation from your healthcare professional

Keep in mind that even if your application is approved, you will still need to pay a deductible. For more information, please refer to the Chronic Disease and Disability Benefits Brochure (PDF 400 KB).
