Make the most of your appointments.

Prepare for your appointment

Prepare for your next appointment with your healthcare team with these three key steps:

1. Make sure your latest sensor glucose data is uploaded to LibreView*

2. Write down any specific questions or concerns you want to discuss with your healthcare team so you feel prepared

3. Consider taking a friend or family member with you to the appointment to ensure you don’t miss anything

At your appointment

Use your glucose data on LibreView* to start the discussion.

Your Ambulatory Glucose Profile (AGP) is a useful tool for you and your healthcare team to make informed decisions about your diabetes management.

A good starting point on LibreView* is the ‘Glucose Patterns Insights’ report. This includes your traffic light ratings.

Questions you may wish to discuss with your healthcare team based on your data:

  • Are there any particular times when I’m more likely to experience hypoglycemia (where the blue wave falls into the low threshold area), and what can I do to address this?

  • How many of my readings align with my glucose goals?

  • How variable are my glucose readings from day to day, and what does this mean?

  • Based on this report, can we talk about my goals?

After your appointment

Make a note of the goals you have discussed with your healthcare team and post them somewhere visible so you can check how you’re doing between appointments. Keep checking your glucose regularly to see patterns and trends and build confidence in reaching goals!

* The LibreView website is only compatible with certain operating systems and browsers. Please check www.libreview.com for additional information.

† To get a complete glycemic picture over the last 3 months, the sensor must be replaced every 14 days and the sensor must be scanned at least once every 8 hours.

‡ Automatic upload requires a wireless internet connection or mobile data connection.
