Is the FreeStyle Libre 2 system covered in Saskatchewan?

Yes! The FreeStyle Libre 2 system is covered under the Saskatchewan Drug Plan for those who meet certain criteria (see more on this below).

Am I eligible for coverage?

You may be eligible for coverage if you are between the ages of 4 and 17, have a prescription for the FreeStyle Libre 2 system and an approved Exception Drug Status Application stating that you meet the following criteria:

  • Are in routine follow up and under the care and management of, or in consultation with, a pediatric diabetes specialist
  • Demonstrate the ability to use the CGM appropriately
  • Demonstrate a reasonable understanding of what the CGM can do and how it can benefit your care
  • Are willing to use the CGM properly and use the data from the technology to make safe and effective diabetes management decisions.

You must also meet ONE of the following criteria:

  • Approved for the Saskatchewan Aids to Indepenent Living (SAIL) Insulin Pump Program (IPP), or
  • On both basal AND bolus insulin, or
  • Have hyperinsulinism requiring frequent blood glucose monitoring

Am I eligible for the Saskatchewan Drug Plan?

What is covered?

Coverage includes both the FreeStyle Libre 2 system sensors and the FreeStyle Libre 2 reader.

Are there any limitations I should know about?

Initial approval for the FreeStyle Libre 2 system is for one year.

 

How do I get reimbursed for the FreeStyle Libre 2 system?

First, talk to your healthcare professional to get a prescription. Your healthcare professional will also need to complete an exception status request form. They will need to include information such as your Health Card number, date of birth, and confirm that you meet the eligibility criteria.

Are there any out-of-pocket costs?

The Special Support Program has a deductible, which is established based on your total family income. There may also be a copayment. Please refer to the Special Support Estimate Calculator to get an estimate of your deductible and copayment.
