Create a Practice ID and invite patients to share their glucose data with you

FreeStyle LibreLink App3 users can automatically share their most recent glucose data with your practice


Set up your LibreView4 Practice

Create a LibreView account and Practice ID so patients can share their glucose data with your practice.

STEP 1

Create your 'Practice ID' or sign into your LibreView Practice account at www.LibreView.com.

STEP 2

Select the Settings Menu icon

STEP 3

Select 'Create a New Practice' then select 'Continue'

STEP 4

Complete the required information, then select 'Create Practice' to create your LibreView Practice and generate your LibreView Practice ID


Option 1: Invite patients to share glucose data with Practice ID

The first option to connect is for you to provide your Practice ID to your
patient. They can enter the Practice ID into the FreeStyle LibreLink App
3 to
enable automatic glucose data sharing8

Locate your Practice ID
  1. Your Practice ID is available under My Practices
  2. My Practice ID: XXXXXXX

 
 
 
 
A patient LibreView4 account is free and secure7

When patients download the FreeStyle LibreLink App3, they'll be asked to to sign in or create a LibreView4 account. A LibreView account is required to share information.

 
Patients enter your Practice ID
  1. Patients can simply follow these steps:
  2. Select the app's 'Menu' icon
  3. Tap 'Connected Apps'
  4. Tap 'LibreView'
  5. Tap 'Connect to Practice'
  6. Enter 'Practice ID' and follow prompts to connect

 

Option 2: Invite patients to share glucose data via email

You can initiate data sharing by sending your patients an email using LibreView4

STEP 1

Search for a patient using the top navigation bar or add a new patient s email address

STEP 2

Select the patient s profile, then select Invite Patient and use the email address associated with their account.

 
STEP 3

Your invitation will appear in the patient's FreeStyle LibreLink App3 and in their email inbox.

Or patients can accept your invitation directly in their app

  3. Tap 'Connected Apps'
  4. Tap 'LibreView'
  5. Complete the prompts to connect


Upload a device

Create a LibreView account and Practice ID so patients can share their glucose data with your practice.

STEP 1

Go to Upload device icon.

Connect the device to your computer with the correct cable

 

STEP 3

Choose upload option

The LibreView device driver s software update is required to upload a device. Click Download the LibreView Device drivers software to update.
