A one-month supply includes two (2) FreeStyle Libre 3 sensors. Your voucher can be redeemed for only one (1) free* sensor. To use it, simply present it to the pharmacist†. He or she will redeem the voucher to cover the cost of one sensor and will charge you or your insurance plan for the other sensor.
Your free* sensor voucher is valid for 60 days from the issue date. The expiration date is listed on each voucher.
SteadyMD offers you an option to request an online prescription for the FreeStyle Libre 3 system from a healthcare professional. Telehealth services through SteadyMD may not be appropriate or permitted under applicable law in all instances. Diabetes patients below 18 years old may not be eligible for SteadyMD services. SteadyMD and the healthcare professionals engaged by SteadyMD are solely responsible for determining that SteadyMD’s telehealth service is in the best interest of the patient and consistent with any and all applicable laws. The use of SteadyMD telehealth services does not guarantee a prescription for the FreeStyle Libre 3 system. SteadyMD may charge a fee of $15 to cover the cost of your telehealth service.
No, you do not have to use the telehealth services from SteadyMD. Please refer to your printed voucher or confirmation email for other options, including obtaining a prescription from your doctor. Remember, a prescription is required to obtain the FreeStyle Libre 3 sensor.
